We are a CalOptima provider; however, our office is currently closed to new members, as reflected in the CalOptima portal. While we do our best to schedule appointments as soon, we cannot guarantee availability within a specific timeframe. If you need more immediate care, we recommend contacting your primary care provider (PCP) to request reassignment to a facility with earlier availability.
A deductible is the amount you are responsible to pay for medical services before your health insurance begins to pay. If your health plan deductible has not been met, we may be required to collect a portion of that payment at each visit.
Coinsurance is a form of cost sharing with your insurance, that requires you to pay a percentage of your medical services after your deductible has been met.
Based on your insurance plan, we may be required to collect a portion of your coinsurance at each visit.
Your copayment amount is based on your contract with your insurance company , that is usually chosen by you or your employer.
